When a tooth has lost approximately half of its structure due to dental caries, prompt clinical intervention is essential to prevent further decay, preserve function, and avoid complications such as pulpitis, abscess formation, or tooth loss.
The appropriate treatment depends on several factors—including the extent and location of the cavity, whether the pulp is involved, the patient’s symptoms, and overall oral health. If the decay remains confined to dentin without pulp exposure or irreversible inflammation, a large composite resin restoration or an inlay/onlay may be indicated. These restorations provide structural support while conserving remaining healthy tooth tissue.
Should caries have progressed to involve the pulp—evidenced by spontaneous pain, prolonged thermal sensitivity, or radiographic signs of pulpal involvement—root canal therapy becomes necessary before definitive restoration. Following successful endodontic treatment, a full-coverage crown is typically recommended to restore strength, seal the tooth, and reduce fracture risk, especially for posterior teeth subjected to high occlusal forces.
In cases where insufficient coronal structure remains to retain a conventional restoration—even after root canal therapy—a post-and-core buildup may be required prior to crown placement. This reinforces the remaining tooth structure and provides adequate retention for the final prosthesis.
Early diagnosis and timely management are critical: delaying care increases the likelihood of more invasive procedures, higher treatment costs, and poorer long-term prognosis. Patients presenting with a tooth compromised by extensive caries should undergo comprehensive evaluation—including clinical examination, diagnostic radiographs, and vitality testing—to determine the optimal, evidence-based treatment pathway.
