Patients with markedly low cholesterol (e.g., total cholesterol < 120 mg/dL or LDL < 50 mg/dL) may report nonspecific symptoms including fatigue, muscle weakness, and decreased concentration. More notably, very low HDL cholesterol (< 30 mg/dL in men or < 35 mg/dL in women) has been linked to increased susceptibility to infections and impaired immune function, as HDL plays a role in modulating inflammation and pathogen clearance. In rare cases, profound hypocholesterolemia may contribute to mood disturbances such as depression or anxiety, potentially due to altered neuronal membrane fluidity and serotonin receptor function.
Clinically significant hypolipidemia is frequently secondary to serious conditions: chronic liver disease (e.g., cirrhosis or primary biliary cholangitis), malabsorption syndromes (e.g., celiac disease or cystic fibrosis), severe malnutrition, advanced malignancy, or inherited disorders such as abetalipoproteinemia or hypobetalipoproteinemia. Certain medications—including high-dose statins, PCSK9 inhibitors, and some antiretroviral agents—can also lower lipid levels substantially, though symptomatic hypolipidemia from pharmacotherapy remains uncommon.
Diagnosis requires comprehensive lipid panel testing alongside evaluation for underlying etiologies—such as liver enzyme assessment, serum albumin, thyroid-stimulating hormone, inflammatory markers, and nutritional indices. Management focuses on identifying and treating the root cause rather than artificially raising cholesterol; lipid supplementation is not indicated unless a specific genetic disorder is confirmed and associated with complications like fat-soluble vitamin deficiency or neuropathy.
