Unlike over-the-counter serums or temporary enhancements, this surgical intervention involves autologous hair follicle transplantation.
The procedure is typically performed under local anesthesia with optional sedation. A donor site—most commonly the occipital scalp—is harvested using either follicular unit extraction (FUE) or, less frequently, strip harvesting with subsequent dissection. Hair follicles are carefully selected for their fine caliber and natural curvature, which closely mimic native eyelashes. Each graft usually contains one to two follicular units to ensure appropriate density and avoid clumping.
Micro-incisions are then made along the upper and/or lower eyelid margin, precisely at the gray line—the anatomical junction between the mucocutaneous and cutaneous zones—using ultra-fine instruments (e.g., 27-gauge needles or custom micro-blades). Grafts are inserted at a shallow angle (10–15 degrees) and oriented parallel to the natural lash growth direction to optimize aesthetic integration and minimize risk of corneal irritation or misdirected growth.
Postoperatively, patients receive topical antibiotic ointment and detailed instructions on lid hygiene, avoidance of rubbing or mechanical trauma, and protection from UV exposure. Initial shedding of transplanted shafts occurs within 2–4 weeks—a normal part of the hair cycle—but permanent regrowth begins at approximately 3–4 months, with full maturation by 9–12 months. Long-term maintenance may include periodic trimming and, in select cases, low-dose topical bimatoprost to support follicular health.
Candidates must undergo thorough ophthalmologic evaluation to exclude contraindications such as active ocular surface disease, severe dry eye, blepharitis, or meibomian gland dysfunction. As with any surgical intervention, potential complications include infection, graft malposition, trichiasis, or transient lid edema—all of which are rare when performed by experienced oculoplastic or oculofacial surgeons.
