Visual acuity—the ability to discern fine detail—is a fundamental measure of eye health and function. In clinical practice, “normal” visual acuity is conventionally defined as 20/20 (in the U.S. and other countries using imperial units) or 6/6 (in metric-using nations such as the UK and Australia). This means that at 20 feet (or 6 meters), a person can correctly identify letters or symbols of a size that a person with typical vision can also recognize at that same distance.
It’s important to clarify that 20/20 does not represent “perfect” vision—it reflects only one aspect of visual function. Other critical components include contrast sensitivity, color vision, peripheral awareness, depth perception, and accommodation (the eye’s ability to focus on near objects). A patient may have 20/20 acuity yet experience significant visual impairment due to conditions like glaucoma, retinitis pigmentosa, or optic neuropathy, which primarily affect side vision or contrast discrimination.
Visual acuity testing is typically performed monocularly (one eye at a time) using standardized charts—most commonly the Snellen chart for adults or age-appropriate alternatives like the Lea Symbols or HOTV chart for young children. Results are recorded as a fraction: the numerator indicates the test distance (e.g., 20 feet), while the denominator reflects the smallest line the patient can read accurately. For example, 20/40 means the patient sees at 20 feet what a person with normal vision sees at 40 feet—indicating reduced acuity.
While 20/20 is the benchmark for standard distance vision, many healthy individuals—especially younger adults—achieve better than 20/20 (e.g., 20/15 or 20/12), reflecting superior resolution. Conversely, visual acuity below 20/20 may be entirely appropriate in certain contexts: infants begin life with acuity around 20/400 and gradually improve over the first several years; older adults often experience mild declines due to presbyopia or early cataract formation, even with otherwise healthy eyes.
Clinically, visual acuity serves as a sensitive screening tool but must always be interpreted alongside comprehensive assessment—including refraction, intraocular pressure measurement, slit-lamp examination, and dilated fundus evaluation—to distinguish refractive errors (e.g., myopia, hyperopia, astigmatism) from pathological conditions affecting the retina, optic nerve, or visual pathways.
