Long-acting reversible contraceptives (LARCs) constitute the cornerstone of highly effective contraception. This category includes intrauterine devices (IUDs)—both copper and hormonal—and contraceptive implants. Hormonal IUDs (e.g., levonorgestrel-releasing devices) provide up to seven years of protection, while the copper IUD offers non-hormonal efficacy for up to ten years. It is important to note that while injectable progestin-only contraceptives (e.g., depot medroxyprogesterone acetate) demonstrate high effectiveness with perfect use, their typical-use failure rate exceeds 1% annually due to delays in repeat injections—placing them just outside the strict “highly effective” threshold in real-world settings.
