Shortness of breath—medically termed dyspnea—is a common yet potentially serious symptom that signals the body’s struggle to meet its oxygen demands. It may manifest as an inability to catch one’s breath, a feeling of tightness or constriction in the chest, or a sensation of air hunger—even at rest or with minimal exertion.
Dyspnea arises from disruptions across multiple physiological systems. Pulmonary causes include obstructive conditions like asthma and chronic obstructive pulmonary disease (COPD), restrictive disorders such as interstitial lung disease or pleural effusion, and acute events like pulmonary embolism or pneumonia. Cardiac contributors range from heart failure and coronary artery disease to arrhythmias and valvular dysfunction—each impairing the heart’s ability to pump oxygenated blood efficiently.
Non-cardiopulmonary origins are also significant: severe anemia reduces oxygen-carrying capacity; anxiety disorders can trigger hyperventilation and subjective breathlessness without objective gas exchange impairment; metabolic acidosis—as seen in diabetic ketoacidosis—stimulates compensatory rapid breathing; and neuromuscular diseases like myasthenia gravis or amyotrophic lateral sclerosis weaken respiratory muscles.
Because dyspnea is a nonspecific symptom with broad differential diagnosis, clinical evaluation must be systematic. Key elements include onset (acute vs. chronic), temporal pattern (exertional, nocturnal, positional), associated signs (e.g., orthopnea, paroxysmal nocturnal dyspnea, edema, wheezing, or cyanosis), and comorbidities. Diagnostic workup often involves pulse oximetry, arterial blood gas analysis, chest imaging, spirometry, electrocardiography, and echocardiography—tailored to suspected etiology.
Importantly, acute-onset dyspnea—especially when accompanied by chest pain, syncope, hemoptysis, or hypotension—requires immediate medical assessment to rule out life-threatening conditions such as massive pulmonary embolism, acute myocardial infarction, or tension pneumothorax.
