Rosacea is a chronic inflammatory skin disorder primarily affecting the central face, characterized by persistent erythema, transient flushing, telangiectasia, and—depending on subtype—papules, pustules, phymatous changes, or ocular manifestations. While its precise etiology remains incompletely understood, current dermatologic research points to a complex interplay of genetic predisposition, dysregulated innate immunity, neurovascular hyperreactivity, and environmental triggers such as ultraviolet radiation, Demodex mites, and certain microbial components.
Although traditional Chinese medicine (TCM) historically described facial redness and swelling using concepts like “lung heat,” “stomach fire,” or “blood stasis,” these frameworks reflect pre-scientific explanatory models rooted in holistic pattern differentiation—not pathophysiological mechanisms validated by modern biomedical science. Contemporary evidence-based dermatology does not support TCM diagnostic categories as causal explanations for rosacea. Instead, robust clinical and molecular studies have identified key contributors including abnormal cathelicidin processing, Toll-like receptor 2 (TLR2) activation, increased matrix metalloproteinase activity, and aberrant sensory nerve signaling that amplifies vasodilation and inflammation.
Effective management relies on evidence-based interventions: topical agents such as ivermectin or metronidazole for inflammatory lesions; oral tetracyclines (e.g., doxycycline at anti-inflammatory doses) for moderate-to-severe cases; and vascular lasers or intense pulsed light for persistent erythema and telangiectasia. Patient education on trigger avoidance—including sun protection, gentle skincare, and identification of individual exacerbating factors—is also foundational to long-term control.
