Pseudomonas aeruginosa pneumonia is a serious, often healthcare-associated infection caused by the Gram-negative bacterium Pseudomonas aeruginosa. This opportunistic pathogen thrives in moist environments—including hospital water systems, ventilator circuits, and respiratory therapy equipment—and is particularly adept at colonizing compromised hosts. Risk factors include prolonged mechanical ventilation, recent broad-spectrum antibiotic use (which disrupts normal flora and selects for resistant strains), immunosuppression (e.g., from chemotherapy or corticosteroids), cystic fibrosis, structural lung disease such as bronchiectasis, and severe underlying illness like advanced COPD or hematologic malignancy.
The organism’s intrinsic resistance to many antibiotics—mediated by low outer membrane permeability, efflux pumps, and inducible β-lactamases—makes it especially challenging to treat. Virulence factors including exotoxin A, type III secretion system effectors, and biofilm formation further contribute to tissue invasion, immune evasion, and persistent infection. In critically ill patients, P. aeruginosa pneumonia frequently arises as a nosocomial or ventilator-associated pneumonia (VAP), carrying high morbidity and mortality rates, particularly when multidrug-resistant (MDR) or extensively drug-resistant (XDR) strains are involved.
