One common cause is bacterial vaginosis (BV), a dysbiosis characterized by depletion of protective Lactobacillus species and overgrowth of anaerobic bacteria such as Gardnerella vaginalis. BV typically presents with thin, grayish-white discharge and a characteristic fishy odor, especially after intercourse, though some individuals report only subjective wetness without overt odor or irritation.
Vulvovaginal candidiasis, particularly recurrent or non-albicans forms, may also manifest atypically—not always with intense pruritus or thick curd-like discharge. In some cases, chronic low-grade inflammation leads to increased transudation and perceived moisture, sometimes accompanied by subtle erythema or fissuring of the vestibule.
Hormonal fluctuations play a significant role: perimenopausal women may experience erratic estrogen levels that impair epithelial maturation and glycogen deposition, resulting in altered mucosal integrity and compensatory secretory changes. Anatomical factors must also be considered: urethral diverticula, incomplete bladder emptying, or stress urinary incontinence may present as vulvar dampness rather than overt urinary leakage—particularly when fluid is absorbed into pubic hair or undergarments before detection. Similarly, fistulous tracts (e.g., vesicovaginal or rectovaginal) require exclusion in refractory cases, especially if associated with foul odor, fecal material, or recurrent urinary tract infections.
Accurate diagnosis hinges on a detailed history—including timing, triggers, associated symptoms (itch, burning, dyspareunia, odor), hygiene practices, and medication use—followed by targeted physical examination and, when indicated, wet-mount microscopy, pH testing, nucleic acid amplification tests (NAATs), or referral for colposcopy or urodynamic assessment.
