Key contributors include prior pelvic inflammatory disease—especially infections caused by *Chlamydia trachomatis* or *Neisseria gonorrhoeae*—which can lead to tubal scarring, adhesions, and impaired ciliary function. Previous tubal surgery, such as sterilization reversal or salpingostomy, may also distort anatomy or compromise peristalsis. Endometriosis, congenital tubal anomalies, and use of intrauterine devices (IUDs) or progestin-only contraceptives are associated with increased relative risk, though absolute risk remains low.
