Amniocentesis is a well-established prenatal diagnostic procedure used primarily to detect chromosomal abnormalities, genetic disorders, and certain neural tube defects. Performed typically between 15 and 20 weeks’ gestation, it involves the transabdominal insertion of a thin needle under real-time ultrasound guidance to withdraw a small volume—usually 15–20 mL—of amniotic fluid containing fetal cells.
While amniocentesis provides highly accurate diagnostic information, it carries a small but quantifiable risk of pregnancy loss. Current evidence from large prospective studies and meta-analyses estimates the procedure-related miscarriage rate at approximately 0.1% to 0.3%—meaning roughly 1 in 300 to 1 in 1,000 procedures may result in fetal loss beyond background risk. This risk is significantly lower than historically reported figures and reflects improvements in ultrasound imaging, operator experience, and procedural technique.
Other potential complications include transient leakage of amniotic fluid (occurring in less than 1% of cases), mild uterine cramping, or vaginal spotting—symptoms that are usually self-limiting. Rarely, infection (chorioamnionitis) or maternal Rh sensitization may occur; for Rh-negative mothers, prophylactic anti-Rh immunoglobulin is routinely administered to prevent alloimmunization.
Importantly, amniocentesis does not increase the risk of preterm labor, birth defects, or long-term neurodevelopmental impairment in the child when performed by trained specialists in appropriate clinical settings. The decision to proceed should be individualized, weighing the diagnostic benefits against procedural risks—and always following comprehensive pretest counseling regarding alternatives such as cell-free DNA screening or chorionic villus sampling.
