Hyaluronic acid is widely used in dermatology, orthopedics, and ophthalmology for its exceptional water-binding capacity and biocompatibility. When administered as a dermal filler, intra-articular injection, or ocular solution, it is generally well tolerated. However, like any medical intervention, it carries potential adverse effects.
Common side effects are typically mild and transient, especially with cosmetic use. These include localized swelling, erythema, tenderness, bruising, and firmness at the injection site—often resolving within a few days to one week. In ophthalmic applications, such as during cataract surgery or dry eye treatment, patients may experience transient ocular discomfort, blurred vision, or mild conjunctival irritation.
Less common but clinically significant complications can occur. With dermal fillers, vascular compromise—such as inadvertent intravascular injection—may lead to skin necrosis, ulceration, or even blindness if retinal artery occlusion occurs. Delayed hypersensitivity reactions, granuloma formation, and biofilm-associated infections have also been reported, particularly with repeated treatments or suboptimal injection technique.
Intra-articular hyaluronic acid injections for osteoarthritis are associated with injection-site pain, joint swelling, and transient arthralgia. Rare systemic reactions—including urticaria, angioedema, or anaphylaxis—are possible, though exceedingly uncommon given hyaluronic acid’s endogenous nature and low immunogenicity.
Patient-specific risk factors—including history of autoimmune disease, coagulopathy, active infection at the injection site, or prior adverse reaction to hyaluronic acid—should be carefully evaluated before administration. Strict adherence to evidence-based protocols, sterile technique, and anatomical knowledge significantly mitigates risks. Patients should be counseled on expected side effects and instructed to seek immediate medical attention for signs of vascular compromise or severe allergic response.
