Parents may occasionally notice a small, painless, non-itchy lump behind their infant’s ear—a finding that commonly prompts concern but is often benign. This type of swelling is most frequently attributable to reactive lymphadenopathy: enlargement of the postauricular lymph node in response to minor, localized immune stimulation—such as a mild scalp rash, transient viral infection, or even routine skin flora exposure. These nodes are normally present and palpable in many healthy infants and young children due to their relatively thin overlying tissue and active immune development.
Other potential causes include sebaceous cysts, which arise from blocked hair follicles or oil glands and typically present as smooth, mobile, subcutaneous nodules; or congenital remnants such as branchial cleft cysts, though these are less common and often appear later in childhood or adolescence. Less likely—but important to recognize—are rare entities like dermoid cysts or, exceptionally, neoplastic processes; however, these are typically associated with additional features such as rapid growth, fixation to underlying structures, skin changes, or systemic symptoms (e.g., fever, weight loss, night sweats), none of which are present in the scenario described.
Clinically, a solitary, soft-to-firm, freely movable, non-tender postauricular nodule measuring under 1.5 cm—and stable over several weeks—is highly suggestive of benign reactive lymphadenopathy. Observation with serial monitoring is appropriate in asymptomatic infants. Evaluation by a pediatrician or pediatric otolaryngologist is recommended if the lump increases in size, becomes tender or erythematous, persists beyond three months, or is accompanied by other concerning signs—including bilateral involvement, systemic symptoms, or failure to thrive.
