Subacute thyroiditis is an inflammatory condition of the thyroid gland, typically triggered by a viral infection. One of its hallmark symptoms is fever—often low- to moderate-grade—but sometimes spiking higher during the acute phase. While fever may resolve spontaneously as the disease progresses, ignoring it entirely is not advisable.
Fever in subacute thyroiditis reflects underlying immune-mediated thyroid inflammation and tissue damage. It frequently coincides with other systemic signs—including fatigue, myalgia, arthralgia, and neck pain that may radiate to the jaw or ears—and often precedes or accompanies thyrotoxicosis due to hormone leakage from damaged follicles. Unaddressed fever can exacerbate discomfort, impair daily function, and obscure clinical monitoring of disease activity.
Management focuses on symptom control and inflammation reduction. Nonsteroidal anti-inflammatory drugs (NSAIDs) are first-line for mild cases; corticosteroids such as prednisone are indicated for persistent or severe symptoms—including refractory fever, significant pain, or marked functional impairment. Beta-blockers may be added temporarily if tachycardia or tremor accompanies thyrotoxicosis.
Clinicians emphasize that fever should never be dismissed as “just part of the illness.” Its persistence, worsening, or association with new neurologic, cardiac, or constitutional symptoms warrants prompt re-evaluation to rule out complications or alternative diagnoses—such as bacterial thyroiditis, lymphoma, or systemic autoimmune disorders.
Close follow-up is essential: serial thyroid function tests (TSH, free T4, total T3), inflammatory markers (ESR, CRP), and clinical assessment guide treatment duration and detect progression to hypothyroidism—a common transient phase before eventual thyroid recovery in most patients.
