Among the most characteristic and commonly observed symptoms in women with hypothyroidism is persistent, unexplained fatigue—often described as profound exhaustion that does not improve with rest or adequate sleep. This fatigue frequently coexists with lethargy, reduced motivation, and impaired concentration, collectively contributing to significant functional impairment in daily life and work performance.
Another hallmark feature is menstrual irregularity. Women may experience oligomenorrhea (infrequent periods), menorrhagia (heavy or prolonged bleeding), or secondary amenorrhea (absence of menses for ≥3 consecutive cycles). These disturbances stem from hypothalamic-pituitary-thyroid axis dysregulation, which interferes with gonadotropin-releasing hormone pulsatility and alters sex hormone-binding globulin levels—ultimately impacting ovarian function and cycle regularity.
Weight gain—particularly disproportionate to caloric intake—and difficulty losing weight despite dietary adherence and exercise are also highly prevalent. This reflects a reduction in basal metabolic rate, diminished thermogenesis, and altered lipid metabolism associated with low circulating thyroid hormone concentrations.
Additional frequent presentations include cold intolerance, dry and coarse skin, brittle nails, thinning hair (especially lateral eyebrow loss), constipation, and depressive mood. In reproductive-age women, untreated hypothyroidism is linked to increased risks of infertility, recurrent pregnancy loss, and adverse obstetric outcomes—including preterm birth and impaired neurocognitive development in offspring.
Clinicians should maintain a high index of suspicion for hypothyroidism in women presenting with this constellation of symptoms—especially when multiple features co-occur. Diagnosis relies on serum TSH measurement, with confirmatory free T4 assessment when indicated. Timely initiation of levothyroxine replacement therapy typically results in marked symptom resolution and restoration of hormonal homeostasis.
