Sea-Bomib (brand name Saismei) is a cholesterol-lowering medication—not an anticoagulant. Its active ingredient, bempedoic acid, inhibits ATP citrate lyase (ACL), an enzyme upstream of HMG-CoA reductase in the hepatic cholesterol biosynthesis pathway. By reducing cholesterol synthesis in the liver, it lowers low-density lipoprotein cholesterol (LDL-C) levels, particularly in patients with hypercholesterolemia or familial hypercholesterolemia.
Importantly, bempedoic acid has no direct effect on platelet function, coagulation cascade proteins (e.g., thrombin, factor Xa), or fibrinolytic activity. It does not prolong prothrombin time (PT), activated partial thromboplastin time (aPTT), or other standard coagulation parameters. Therefore, it offers no antithrombotic benefit and should not be used for stroke prevention, deep vein thrombosis prophylaxis, or any other indication requiring anticoagulation or antiplatelet therapy.
Clinical trials—including the CLEAR Outcomes study—demonstrated that bempedoic acid significantly reduced major adverse cardiovascular events (MACE), including myocardial infarction and coronary revascularization, primarily through LDL-C reduction and associated plaque stabilization. However, this cardiovascular benefit is mechanistically distinct from anticoagulation; it reflects improved atherosclerotic disease control, not inhibition of thrombus formation.
Patients requiring both lipid management and thromboprophylaxis must receive appropriately prescribed, evidence-based therapies for each indication—such as statins or PCSK9 inhibitors for dyslipidemia, and warfarin, DOACs, or antiplatelet agents like aspirin or clopidogrel for thrombosis prevention—under close clinical supervision.
