“Sleeping wrinkles”—more accurately termed the *orbicularis oculi muscle ridge*—are subtle, soft, horizontal convexities that appear just below the lower eyelid margin, typically becoming more noticeable when smiling or squinting. This ridge results from the natural anterior projection of the pars orbitalis of the orbicularis oculi muscle, a normal variant present in many individuals. It is not associated with aging, fluid retention, or pathology; rather, it reflects healthy, well-defined muscular anatomy and may even be perceived as aesthetically pleasing due to its contribution to a youthful, alert appearance.
In contrast, true *lower eyelid bags* (or periorbital fat prolapse) arise from age-related weakening of the orbital septum—the thin fibrous membrane that normally confines orbital fat behind the lower eyelid. As this barrier deteriorates, orbital fat herniates forward into the subcutaneous space, producing persistent, soft, often bilateral fullness beneath the eyes. Unlike the orbicularis ridge, eye bags are typically visible at rest, may worsen with fatigue or salt intake, and can be accompanied by shadowing, skin laxity, or tear-trough deformity.
