While spontaneous resolution can occur over 1–2 weeks, patients frequently seek rapid symptomatic relief.
Initial management focuses on conservative measures to reduce inflammation and alleviate discomfort. Warm sitz baths—soaking the perianal area in warm (not hot) water for 10–15 minutes, two to three times daily—promote vasodilation, improve local circulation, and help decrease edema. Over-the-counter topical agents containing hydrocortisone or lidocaine may provide temporary relief from itching and pain, though they do not accelerate clot resorption.
Oral nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen or naproxen, are recommended for their dual anti-inflammatory and analgesic effects. Stool softeners (e.g., docusate sodium) and increased dietary fiber—paired with adequate hydration—are essential to prevent straining during defecation, which can exacerbate symptoms or precipitate recurrence.
For patients presenting within 48–72 hours of symptom onset and experiencing severe pain, office-based excision under local anesthesia is considered the most effective intervention for rapid symptom resolution. This procedure involves elliptical incision over the thrombus, evacuation of the organized clot, and minimal wound management—typically without suturing. Studies show marked pain reduction within hours and significant improvement in swelling within 24–48 hours post-procedure.
It is important to note that delayed presentation (>3–5 days) generally precludes surgical intervention, as the clot begins to organize and inflammation subsides; in these cases, conservative therapy remains the mainstay. Patients should be counseled on long-term prevention strategies—including sustained high-fiber intake, regular physical activity, and avoidance of prolonged sitting or straining—to reduce recurrence risk.
