Effective management begins with accurate diagnosis: true under-eye bags are typically caused by herniation of orbital fat pads through weakened orbital septum, whereas puffiness due to edema or allergic shiners requires different interventions. A thorough clinical evaluation—including assessment of skin elasticity, volume loss, pigmentation, and presence of tear trough deformity—is essential before recommending treatment.
Conservative, non-invasive strategies form the first line of management. These include optimizing sleep hygiene (7–9 hours per night), reducing dietary sodium, managing seasonal or perennial allergies with intranasal corticosteroids or antihistamines, and using cold compresses to transiently reduce vascular congestion and fluid accumulation.
