This disorder often develops silently over years, frequently linked to persistent Helicobacter pylori infection, autoimmune mechanisms (less commonly in the antrum than in the corpus), long-standing bile reflux, or chronic use of nonsteroidal anti-inflammatory drugs (NSAIDs). Patients may remain asymptomatic or report nonspecific symptoms such as epigastric discomfort, early satiety, bloating, or mild nausea—symptoms that overlap significantly with functional dyspepsia and can delay diagnosis.

Management centers on identifying and eliminating modifiable risk factors—most critically, eradicating H. pylori with appropriate antibiotic regimens—and implementing surveillance strategies tailored to individual risk. For patients with C-1 atrophy and no intestinal metaplasia or dysplasia, routine endoscopic surveillance is generally not recommended; however, clinicians should reassess risk periodically and consider repeat endoscopy if symptoms evolve or new alarm features emerge. Lifestyle counseling—including smoking cessation, moderation of alcohol intake, and avoidance of excessive salted or smoked foods—also forms part of evidence-based preventive care.
