The most immediate life-threatening event is loss of consciousness due to cerebral hypoxia—insufficient oxygen delivery to the brain—which typically begins within 15–30 seconds after submersion if breathing is obstructed. If the diver is unable to surface and resume airway patency, cardiac arrest may follow within 2–3 minutes as hypoxemia progresses to bradycardia, ventricular arrhythmias, and ultimately asystole.
Several variables influence the timeline: water temperature (cold water immersion may trigger the mammalian dive reflex, transiently preserving oxygenation but also increasing risk of sudden cardiac events), depth and force of impact (which may cause spinal cord injury, cervical fracture, or traumatic brain injury leading to rapid incapacitation), and pre-existing medical conditions such as cardiac arrhythmias or seizure disorders. In cases of shallow-water diving—particularly head-first entry into water less than 1.5 meters deep—the risk of catastrophic cervical spine injury is high, and death may occur almost instantaneously due to high cervical cord transection or brainstem compression.
It is critical to emphasize that survival is possible even after prolonged submersion, especially in cold water (<15°C), where hypothermia-induced metabolic suppression can extend the window for successful resuscitation. Documented cases exist of neurologically intact recovery after submersion exceeding 30 minutes in icy water. Nevertheless, prompt recognition, immediate rescue, and initiation of high-quality CPR remain the strongest predictors of favorable neurological outcome.
