These point-of-care tests detect antibodies against *Treponema pallidum*, the spirochete responsible for syphilis, typically using whole blood, serum, or plasma. While many FDA-cleared or WHO-prequalified rapid tests demonstrate high sensitivity (90–98%) and specificity (95–99%) in controlled settings, real-world performance can be significantly lower due to user error, improper sample collection, expired reagents, or testing during the seronegative window period—especially in early primary syphilis, when antibody titers may still be undetectable.
Importantly, no rapid test replaces confirmatory laboratory evaluation. A reactive result requires follow-up with treponemal-specific testing (e.g., TP-PA or chemiluminescence immunoassay) and nontreponemal testing (e.g., RPR or VDRL) to assess disease activity, stage, and treatment response. Conversely, a negative rapid test does not rule out syphilis—particularly in individuals with recent exposure (<2–4 weeks), immunosuppression, or very early infection. Clinical correlation remains essential: patients with suspicious lesions (e.g., painless chancre, mucocutaneous rash, or neurologic symptoms) warrant immediate referral for nucleic acid amplification testing (NAAT) of lesion exudate or cerebrospinal fluid, alongside standard serology.
Public health guidelines—including those from the CDC and WHO—recommend that rapid syphilis tests be deployed only within structured programs with trained personnel, quality assurance protocols, and linkage-to-care pathways. Self-testing without access to timely counseling, confirmatory diagnostics, and treatment undermines both individual outcomes and broader syphilis control efforts. For optimal accuracy and patient safety, clinicians should interpret rapid test results in context—not as standalone diagnoses—but as screening tools that initiate, rather than conclude, the diagnostic process.
