First-line management for early-stage onychomycosis typically involves topical antifungal agents. Prescription-strength ciclopirox or efinaconazole nail lacquers are FDA-approved for mild to moderate infections confined to less than 50% of the nail plate and without involvement of the nail matrix. These formulations penetrate keratinized tissue to inhibit fungal growth, but require consistent daily or twice-weekly application for up to 48 weeks—adherence is critical for efficacy.
In cases where topical therapy is insufficient—or when infection involves the proximal nail fold, lunula, or multiple nails—oral antifungals such as terbinafine or itraconazole become appropriate. Adjunctive measures support pharmacologic treatment: regular nail debridement by a podiatrist or dermatologist helps reduce fungal burden and enhances topical drug penetration; keeping feet dry and wearing moisture-wicking socks minimizes the warm, humid environment fungi thrive in; and avoiding shared footwear or communal bathing areas lowers reinfection risk.
Because clinical appearance alone can be misleading—psoriasis, lichen planus, or trauma may mimic onychomycosis—diagnostic confirmation via potassium hydroxide (KOH) preparation, fungal culture, or polymerase chain reaction (PCR) testing is recommended before initiating systemic therapy. Accurate diagnosis ensures appropriate treatment selection and avoids unnecessary medication exposure.
