Athlete’s foot is caused by dermatophyte fungi, most commonly *Trichophyton rubrum* or *Trichophyton mentagrophytes*, which thrive in warm, moist environments. While sweat itself is sterile and does not harbor fungi, persistent moisture from hyperhidrosis creates ideal conditions for fungal colonization and proliferation—particularly between the toes and on the soles.
Individuals with chronic plantar hyperhidrosis often experience maceration (softening and whitening) of the skin, microfissures, and compromised epidermal barrier function—all of which facilitate fungal invasion. Additional risk factors include wearing occlusive footwear, sharing contaminated surfaces (e.g., locker room floors or communal showers), and inadequate foot hygiene.
It’s important to distinguish athlete’s foot from other causes of foot discomfort that may co-occur with hyperhidrosis—such as contact dermatitis, bacterial superinfection (e.g., erythrasma or cellulitis), or pitted keratolysis. Accurate diagnosis typically requires clinical evaluation and, when uncertain, potassium hydroxide (KOH) microscopy or fungal culture.
Management focuses on both antifungal therapy—topical azoles, allylamines, or ciclopirox—and addressing underlying hyperhidrosis through measures such as aluminum chloride hexahydrate solutions, iontophoresis, or, in refractory cases, botulinum toxin injections. Patient education on daily foot drying, moisture-wicking socks, and regular footwear rotation remains a cornerstone of prevention.
