Tooth powders typically contain abrasive agents such as calcium carbonate, baking soda (sodium bicarbonate), or silica, along with flavorings and sometimes low concentrations of peroxide or activated charcoal. Their primary mechanism is mechanical abrasion: they physically scrub away surface stains on enamel. As such, they may produce modest lightening of extrinsic discoloration caused by coffee, tea, tobacco, or certain foods—provided the enamel remains intact and the product is used sparingly.
However, tooth powders do not address intrinsic staining—such as that resulting from tetracycline exposure during tooth development, fluorosis, or age-related dentin darkening. Moreover, excessive or prolonged use can lead to enamel erosion, increased tooth sensitivity, and heightened susceptibility to caries due to loss of protective mineral structure.
Unlike professionally supervised whitening treatments—which use controlled concentrations of carbamide or hydrogen peroxide to penetrate enamel and oxidize chromogens—over-the-counter powders lack standardized formulation, regulatory oversight (in many jurisdictions), and evidence-based efficacy data. Clinical studies supporting significant, lasting whitening outcomes with tooth powders are notably absent from peer-reviewed dental literature.
Dental professionals recommend evidence-based alternatives: ADA-accepted whitening toothpastes (which combine mild abrasives with polishing agents), custom-fitted tray-based peroxide gels, or in-office power bleaching under supervision. Patients with existing restorations, gum recession, or enamel hypoplasia should consult a dentist before initiating any whitening regimen, as inappropriate use may compromise oral health rather than enhance aesthetics.
