Genetic Mutation Testing in Hematology is a specialized diagnostic procedure used to identify acquired or inherited DNA alterations in blood cells and hematopoietic stem cells that underlie hematologic disorders—including myeloid malignancies (e.g., acute myeloid leukemia, myelodysplastic syndromes, chronic myeloid leukemia), lymphoid neoplasms (e.g., chronic lymphocytic leukemia, lymphomas), inherited bone marrow failure syndromes (e.g., Fanconi anemia, dyskeratosis congenita), and thrombophilias or coagulopathies with genetic origins. Unlike general genetic screening, hematologic mutation testing focuses on somatic variants (e.g., FLT3, NPM1, IDH1/2, JAK2, CALR, MPL, BCR-ABL1, TP53) and germline pathogenic variants (e.g., RUNX1, GATA2, CEBPA, DDX41) with direct implications for diagnosis, risk stratification, therapeutic selection (e.g., targeted inhibitors like midostaurin for FLT3-mutated AML), transplant eligibility, and familial counseling. Pathogenesis involves clonal expansion of mutated hematopoietic progenitors due to driver mutations disrupting signaling pathways (e.g., JAK-STAT), epigenetic regulation, splicing machinery, or tumor suppression—leading to dysregulated proliferation, impaired differentiation, genomic instability, or apoptosis resistance. Epidemiologically, somatic mutations are highly prevalent in hematologic cancers: >90% of AML cases harbor at least one clinically relevant mutation; JAK2 V617F occurs in ~95% of polycythemia vera patients; and germline predisposition accounts for ~5–10% of pediatric AML and younger adult MDS cases. Risk factors include advanced age (for somatic mutations), prior chemotherapy/radiation (therapy-related myeloid neoplasms), family history of hematologic malignancy or cytopenias, and exposure to benzene or ionizing radiation. While the test itself is non-invasive (typically using peripheral blood or bone marrow aspirate), its clinical impact profoundly affects quality of life: timely detection enables precision therapy—reducing ineffective chemotherapy, minimizing relapse risk, and avoiding unnecessary allogeneic transplants—yet delays or misinterpretation may lead to anxiety, treatment escalation, or psychosocial burden from uncertain germline findings. Patients often experience improved QoL when results guide effective targeted interventions, but also face challenges related to genetic stigma, insurance implications, and familial disclosure dilemmas—necessitating integrated genetic counseling within hematology care.
