This results in elevated portal venous pressure (>10 mmHg), triggering the formation of portosystemic collateral vessels—most critically esophageal and gastric varices. These fragile, dilated submucosal veins lack supportive tissue and are prone to rupture under hemodynamic stress (e.g., transient increases in intra-abdominal or intrathoracic pressure), causing acute, often massive upper GI bleeding. Less commonly, bleeding may originate from portal hypertensive gastropathy or ectopic varices (e.g., duodenal, colonic). Epidemiologically, approximately 5–15% of patients with compensated cirrhosis develop varices annually; among those with established varices, the annual risk of first bleed is 5–15%, rising to 30–40% in those with large varices or red wale signs. Mortality remains high: 15–20% within 6 weeks of an initial episode, with rebleeding occurring in 60% of untreated survivors within 1 year. Key risk factors include Child-Pugh class C or MELD score ≥15, large varices, active alcohol use, coagulopathy (low platelets, prolonged INR), bacterial infection (e.g., spontaneous bacterial peritonitis), and ongoing portal hyperdynamic circulation. Beyond acute mortality, recurrent bleeding profoundly impairs quality of life—patients experience persistent fatigue, anxiety about sudden hemorrhage, dietary restrictions (e.g., avoidance of coarse foods), reduced physical activity, social withdrawal, and significant caregiver burden. Hospitalizations, endoscopic surveillance, and long-term pharmacotherapy (e.g., non-selective beta-blockers) contribute to psychological distress and financial strain. Early diagnosis via screening gastroscopy in newly diagnosed cirrhosis is standard of care; management hinges on multidisciplinary coordination between hepatology, gastroenterology, interventional radiology, and critical care. Without timely intervention—including endoscopic band ligation, pharmacologic vasoconstriction (terlipressin/octreotide), antibiotics, and definitive portal pressure reduction (e.g., TIPS)—outcomes deteriorate rapidly. Patient education on symptom recognition (e.g., hematemesis, melena, syncope), strict adherence to prophylactic therapy, and abstinence from alcohol and NSAIDs are essential components of holistic care.
