疾病概述
Diabetic foot is a serious, limb-threatening complication of diabetes mellitus characterized by infection, ulceration, and/or deep tissue destruction in the foot, often occurring in the context of peripheral neuropathy and/or peripheral artery disease. It arises from a complex interplay of metabolic, vascular, neurological, and biomechanical abnormalities. Chronically elevated blood glucose levels damage peripheral nerves (causing sensory, motor, and autonomic neuropathy), impairing protective sensation and foot muscle function—leading to unrecognized trauma and abnormal pressure distribution. Concurrently, hyperglycemia accelerates atherosclerosis and microvascular dysfunction, reducing blood flow to the feet and compromising wound healing and immune defense. Infection—often polymicrobial—can rapidly progress from superficial cellulitis to osteomyelitis or necrotizing fasciitis if untreated. Epidemiologically, diabetic foot ulcers affect approximately 15–25% of people with diabetes over their lifetime; globally, one lower-limb amputation occurs every 30 seconds due to diabetes-related complications. In China, prevalence is rising in parallel with the national diabetes epidemic—estimated at over 140 million adults living with diabetes—and diabetic foot accounts for >60% of non-traumatic lower-limb amputations in endocrinology departments. Key modifiable risk factors include poor glycemic control (HbA1c >8%), smoking, uncontrolled hypertension, dyslipidemia, history of prior foot ulcer or amputation, visual impairment, renal insufficiency, and inadequate footwear or foot hygiene. Non-modifiable risks include longer diabetes duration (>10 years), older age, male sex, and certain genetic predispositions. Beyond physical morbidity, diabetic foot profoundly impairs quality of life: patients report severe pain, mobility limitations, social isolation, depression, anxiety, loss of employment, and caregiver burden. Chronic wounds often necessitate prolonged offloading, frequent clinic visits, and hospitalizations—disrupting daily routines and eroding independence. Psychological distress correlates strongly with ulcer recurrence and delayed healing. Early detection through structured foot screening (monofilament testing, Doppler assessment, dermatological inspection) and multidisciplinary intervention—including endocrinologists, podiatrists, vascular surgeons, infectious disease specialists, and wound care nurses—is critical to prevent progression and preserve limb and life.
