These anomalies include aneuploidies (e.g., trisomy 21, monosomy X), polyploidies, deletions, duplications, inversions, translocations, and ring or marker chromosomes. In reproductive medicine, chromosomal abnormalities are a leading biological cause of recurrent pregnancy loss, implantation failure, unexplained infertility, and congenital disorders. Pathogenically, they stem from errors in meiotic or mitotic division—such as nondisjunction, premature centromere separation, or defective recombination—often exacerbated by advanced parental age (especially maternal age >35), environmental mutagens (e.g., ionizing radiation, chemotherapy), genetic predisposition (e.g., balanced translocation carriers), or impaired DNA repair mechanisms. Epidemiologically, approximately 0.6% of live births harbor a clinically significant chromosomal abnormality; however, the prevalence rises dramatically in adverse reproductive outcomes: ~50–60% of first-trimester miscarriages and up to 8–12% of couples with recurrent pregnancy loss carry detectable cytogenetic anomalies. Risk factors extend beyond maternal age to include paternal age >45, prior child or pregnancy with chromosomal disorder, history of infertility requiring assisted reproductive technology (ART), exposure to gonadotoxins, and certain autoimmune or thrombophilic conditions. Importantly, balanced structural rearrangements (e.g., Robertsonian or reciprocal translocations) may be asymptomatic in carriers but confer high risks of unbalanced gametes—leading to repeated losses or affected offspring. The quality-of-life impact is profound and multifaceted: patients often experience prolonged emotional distress, anxiety, grief, marital strain, financial burden from repeated ART cycles, and social stigma surrounding infertility or pregnancy loss. Many undergo years of diagnostic uncertainty before karyotyping or chromosomal microarray analysis confirms a diagnosis—delaying appropriate counseling and management. Psychosocial support, genetic counseling, and personalized reproductive planning are integral to care. While chromosomal abnormalities themselves cannot be 'cured' post-conception, preimplantation genetic testing for aneuploidy (PGT-A) or structural rearrangements (PGT-SR), donor gametes, prenatal diagnosis with selective termination (where legally and ethically permissible), or natural conception with comprehensive prenatal screening significantly improve reproductive outcomes and psychological resilience.
